Manufacturer co-pay cards (Eli Lilly Zepbound/Mounjaro, Novo Nordisk Wegovy/Ozempic) cap at roughly $1,300 to $1,800 per calendar year. Here is what happens when that limit hits, and how to transition to the cheapest legal cash route.
How Savings Card Caps Work
Savings cards are commercial co-pay subsidies provided by drug manufacturers. They are not insurance, and they are legally prohibited for patients with Medicare, Medicaid, or any federal healthcare program.
Card pays up to ~$150–$200 per 28-day fill. If your co-pay is $200, you pay $25. The subsidy spreads across the full year (resets Jan 1).
Card pays a fixed discount (~$450 off list), leaving you with ~$550–$650/mo. The $1,300–$1,800 cap burns out rapidly in Month 3 or 4.
Ranked Routes After Card Exhaustion
When the card runs out, do not pay full retail list price (~$1,086/mo). Switch to manufacturer-direct self-pay channels:

- 60 Days Before Exhaustion: Track Your Spent Balance
Ask your dispensing pharmacy for the cumulative manufacturer benefit paid to date, or check your card portal. Plug the remaining balance into WalletNoise to project the exact exhaustion fill.
- 30 Days Before Exhaustion: Set Up Direct Accounts
Create an account with LillyDirect / GiftHealth or NovoCare Pharmacy Solutions. Have your prescriber send a separate prescription to the direct pharmacy channel.
- Plan Refills for the 45-Day Continuity Window
For LillyDirect higher doses (7.5 mg to 15 mg), order refills within 45 days of the previous fill to maintain the $449/mo rate instead of escalating to $499 or $699.
- January 1: Re-Enroll for the Annual Reset
Savings card terms and annual caps reset every January 1. Download a new savings card on January 1 to restart the co-pay subsidy for the new benefit year.
